Stephen Milam, PharmD, BCEMP, BCCCP is a dual board-certified clinical pharmacy specialist with more than 15 years of specialized practice at the intersection of emergency medicine and critical care. He currently serves as Emergency Medicine & Critical Care Clinical Pharmacy Specialist at Lewis Gale Medical Center in Salem, Virginia, where he provides direct patient care in both the Emergency Department and ICU. Dr. Milam holds Board Certifications in both Critical Care Pharmacy (BCCCP) and Emergency Medicine Pharmacy (BCEMP) from the Board of Pharmacy Specialties — a combination held by very few pharmacists nationally. His clinical work encompasses the full spectrum of high-acuity pharmacotherapy including sepsis management, toxicology, anticoagulation, hemodynamic support, sedation and analgesia, antimicrobial stewardship, and emergency resuscitation. He currently serves on the Stroke and Sepsis Committees at his institution, and previously served on the Code Blue and Antimicrobial Stewardship Committees. His academic background includes a B.S. in Biochemistry from Virginia Tech and a PharmD from Virginia Commonwealth University's Medical College of Virginia, both completed cum laude, followed by a PGY-1 pharmacy practice residency at Carilion Roanoke Memorial Hospital. Dr. Milam provides expert review and testimony services for plaintiff and defense attorneys, insurance carriers, and workers' compensation matters. Services include comprehensive medical record review, medication error analysis, adverse drug event causation and attribution, standard of care opinions, pharmacokinetic and drug interaction analysis, and deposition and trial expert witness testimony. Because he maintains an active clinical practice, his opinions reflect current standards — not outdated textbook knowledge. Dr. Milam is available for retention on a case-by-case basis. He works with both plaintiff and defense counsel and accepts or declines cases at his discretion. Inquiries may be directed to stephen.milam@gmail.com or by phone at 434-250-2243.
