My name is Nathan Freeman, MD, MBA. I am a board-certified Emergency Medicine physician and currently serve as the Medical Director for both a high-volume community Emergency Department (45,000 annual visits) and a freestanding Emergency Department (15,000 annual visits). I have over 15 years of experience practicing Emergency Medicine, having personally evaluated and treated over 50,000 patients across diverse acuity levels and clinical presentations. My dual role as both a clinician and medical director allows me to bring a comprehensive understanding of emergency care delivery, documentation, systems-based practices, and risk mitigation. As Medical Director, I am responsible for provider performance oversight, APP supervision, operational efficiency, and quality improvement initiatives. I have led initiatives in improving throughput, reducing door-to-doctor time and admit LOS, implementing structured handoff tools, and optimizing patient experience. A typical clinical shift involves the care of 20–30 patients, including medical, trauma, psychiatric, and procedural cases. Presentations range from chest pain, sepsis, stroke, abdominal pain, and shortness of breath to trauma from motor vehicle accidents, falls, lacerations, fractures, and more. Critical patients with life-threatening conditions are prioritized for rapid assessment and intervention, while simultaneously managing other patients in various stages of evaluation or treatment. This demands precise clinical decision-making, excellent time management, and leadership in high-stakes, high-liability environments. Care for each patient includes: 1. History and physical exam 2. Diagnostic evaluation (labs, imaging, bedside ultrasound) 3. Formulation and execution of a treatment plan 4. Monitoring and reassessment 5. Final disposition decision—discharge, admission, or transfer—with clear documentation and patient/family communication Based on my extensive experience, I offer expert witness services including case review, chart analysis, and testimony for both plaintiff and defense in civil and criminal matters. I provide objective, timely, and thorough assessments related to Emergency Medicine standard of care, missed diagnoses, treatment delays, sepsis care, patient handoffs, documentation review, and more.
